HR+/HER2- advanced/metastatic breast cancer: first-line with aromatase inhibitor; post-endocrine with fulvestrant. Standard starting dose.

[Mechanism of Action]

First-in-class highly selective CDK4/6 inhibitor. Prevents Rb phosphorylation, inducing G1 cell cycle arrest.

[Dosage]

125mg daily with food, 21 days on/7 off. Reduce to 100mg or 75mg for toxicity.

[Adverse Reactions]

Neutropenia G3-4 (66%), leukopenia, infections, fatigue, nausea, stomatitis, anemia, alopecia. Febrile neutropenia (2%), PE (2-5%).
