Indications

First-line EGFR mutation-positive NSCLC (exon 19 del, L858R).

Squamous NSCLC after platinum therapy.

Uncommon EGFR mutations.

Mechanism of Action

Irreversible covalent ErbB family (EGFR/HER2/ErbB4) blocker.

Sustained target inhibition with broad spectrum coverage vs first-gen reversible TKIs.

Dosage and Administration

40mg once daily on empty stomach.

Escalate to 50mg if tolerated.

Reduce to 30mg then 20mg for toxicity.

Adverse Reactions

Diarrhea (95% G1-3 22%), rash (89%), stomatitis (71%), paronychia.

ILD (1-2%, 0.5% fatal), hepatotoxicity, keratitis.
