[Indications]

1. Advanced non-squamous NSCLC with cisplatin first-line or maintenance. 2. Malignant pleural mesothelioma with cisplatin.

[Mechanism of Action]

Multi-targeted antifolate inhibiting TS, DHFR and GARFT - key folate-dependent enzymes for nucleotide biosynthesis. Disrupts DNA replication.

[Dosage]

500mg/m2 IV day 1 q21d with cisplatin. Mandatory folic acid 400-1000mcg daily + vitamin B12 1000mcg IM q9w. Dexamethasone premedication.

[Adverse Reactions]

Neutropenia, anemia, fatigue, nausea, stomatitis. Bone marrow suppression, renal failure, interstitial pneumonitis.
